accordance with law do hereby depose and say:

1. That I am duly appointed as Authorized Representative of FeathersProject

Foundation Inc. (herein referred to as the “Company”), with office address at 10/F
Unit 1002 Coherco Financial Tower, Madrigal Business Park, Ayala Alabang,
Muntinlupa City, 1780.

2. In furtherance thereof, the Company submits its Application for Permit to Use Manual
Loose-leaf BOOKS OF ACCOUNTS to the Bureau of Internal Revenue specifying
the following:

a) General Journal
b) General Ledger
c) Cash Receipts Journal
d) Cash Disbursement Journal

3. I commit to permanently bind these loose-leaf forms within fifteen (15) days after the
end of each taxable year or upon the termination of its use.

4. I execute this sworn statement to attest to the truthfulness and veracity of the
foregoing statement and for all legal intents or purposes this may serve.
