AFFIDAVIT

I, SUSANA J. BAÑ ADERA, of legal age, married, a Filipino and a resident of 187
Magsaysay Drive Loakan Proper, Baguio City having been duly sworn in accordance with
law, hereby depose and state:

1. That in our Certificate of Marriage as recorded in the Office of the Civil Registrar
of Baguio and as issued Philippine Statistics Authority it is recorded that my
husband, Efren A. Bañ adera, and I got married on 25 August 1997;

2. That when I secured a copy of our Certificate of Marriage from the National
Statistics Office, Baguio (now Philippine Statistics Authority), and after
scrutinizing the same, I discovered that the recorded date of birth of my husband
appearing therein is 26 January 1966, instead of 26 January 1967;

3. That as recorded in my husband’s Certificate of Birth, his date of birth is 26

January 1967;

4. That I am executing this Affidavit to attest to the truth of the foregoing facts and
to use the same for whatever legal purpose it may serve;

5. Affiant further sayeth naught.
