Sub: - Preliminary failure investigation report of broken power assembly of Loco

For the above said locomotive, new power assemblies received from DLW except carrier
assemblies were fitted. The post yearly attention was done on 02.02.2024 and the loco was
failed on 05.02.2024 (3days). During the investigation, no beach mark or old flaw was found on
the power assembly components. 6th & 14th cylinder liner broken (Fig.1), Pistons broken (Fig.
2&3), P pipe area, thrust washer damaged. No scoring and ceased mark on the piston and liner.
14th location connecting rod was in bend condition and was hit at its centre and heat mark was
noticed (Fig.4), 6th location connecting rod has heat marks in side the fork area (Fig.5 & 6). 14th
location thrust washer was broken into pieces. 14th location connecting rod cap was broken at
serration end (Fig.7). 6th location crank pin having hit marks (Fig. 8).
