I. OBJECTIVES At the end of this lesson, the learners will be able to:
1. Use the appropriate form in requesting for electrical tools, supplies
and materials for a specific job.
A. Content Standards
The learner demonstrates an understanding of the concepts in the preparing
electrical materials and tools using the different forms in electrical installation
and maintenance.

B. Performance The learner independently prepares appropriate electrical materials and tools
Standards using the different forms in electrical installation and maintenance based on
industry standards.

IV. PROCEDURES

e. Introduction What I need to know?

 Learning objectives will be introduced to guide the learners on the


learning targets.
 The learners will identify the needed materials and tools listed as per
job requirement.
 The learners were able to request materials and tools are according to
the list prepared.

What’s new?
 Before beginning the reading session, the learners will be given a Pre-
test to check how much they know about the topic. They may check
their answer against the Answer Key.
 If the learner got 99 to 100% of the items correctly, they may proceed to
the next Lesson. This means that they need not go through the
Lesson because they already know what it is all about. If they failed to
get 99 to 100% correctly, they should go through the Lesson again and
review especially those items which you failed to get.
 The learners will proceed to the Information Sheets. An Information
Sheet contains important notes or basic information that the learners
need to know.
 Upon reading the information sheet, the learners will be familiarized
with the different forms use in requesting, inventory, borrowing and the
job order of the materials and tools in electrical.

 After that, the learners will answer the 10-item quiz to find out their
previous knowledge about the different forms use in requesting,
inventory, borrowing and the job order of the materials and tools in
electrical. They may check their answers on latter part of the module to
determine their background knowledge about the given topic reflected
in the MELC.

Activity 1- Accomplish the table.
 The learners will complete the table, they will identify different forms
use in requesting, inventory, borrowing and the job order of the
materials and tools in electrical and give its description and purpose.

What is it?
 The learners read and understand the concepts they need to know
about requesting appropriate electrical supplies materials and tools
applicable to a specific job.
 After reading the information sheet, the learners are able to answer the
activities in the learning modules.

 In expressing or creating bits of information reflective of what they have
learned during the Development and Engagement phases, the learners
will answer the questions focusing on the given topic. The given
questions are directed on synthesizing their learned information based
on the given content.
 They are expected to relate their learned concepts to the job
requirement

 The learners will answer the questions provided.
 They may check their answers using the key answer found at the latter
part of the module.
