I, MONET LOPERA PIELAGO, of legal age, Filipino citizen, a resident of No. 2386
Alberto St. Angeles Ext. Gen. T. De Leon, Valenzuela City, after having been sworn to in
accordance with law do hereby deposes and states, that:

1. I am the mother of AIMAR ERROL PIELAGO SISON who was born on 5TH day of
OCTOBER 2005, at VALENZUELA CITY;

2. I am giving my consent to my son AIMAR ERROL PIELAGO SISON to apply for a

Student Driver’s Permit at the office of Land Transportation Office;

3. I am executing this affidavit to attest to the truth of the foregoing facts and to inform
Land Transportation Office and other authorities whatever legal matter it may
serve;
