AFFIDAVIT OF REQUEST FOR RECLASSIFICATION

That I, MILAGROS ANDRADA FABIA, of legal age, Filipino citizen and

resident of Brgy. Maasin, Mangaldan, Pangasinan, after having duly sworn to
in accordance with law hereby depose and say;

That I am the only heir of the late CANDIDA ERASQUIN of a certain

parcel of land located in Anolid, Mangaldan, Pangasinan with an area of 590
square meters declared under Tax Declaration No. 004 00188;

That said land is used as fishpond but already in use as a residential

land with an area of 590 square meters.

That I am requesting the Provincial Assessor’s/Municipal Assessor’s

Office of Mangaldan, Pangasinan to re-classify, convert and consolidate a
parcel of land with an area of 590 square meters of the said land from
FISHPOND to RESIDENTIAL LAND.

That I am executing this affidavit to attest the truth of all the foregoing
statement for whatever legal intent it may serve and purposes;
