Subject:- Submit COMPOSITE CLAIM FORM for Part Payment Withdrawal

Sir,
My Name is MOHAMMED NASIM HALIM KHAN I have done final settlement in
member id :- MHBAN01282050000000177 On 15-02-2021 At that time I had
withdrawn my PF money by filling form 19 but at that time I had not filled form 10c due
to which my pension amount has not been withdrawn.

Then after that I currently work in one more company(KDMAL19706670000010211 )

WASTE CARE (JV).

So still my pension money is in 1st company which I am unable to withdraw online or

transfer online. Because pension can be withdrawn only from the last company

I had also requested to transfer the pension money of my old company to my current
company And the amount transfer is also not happening, it is written as claim already
settled because I have filled form 19 and withdrawn the amount of PF.

Therefore, I Humble request to you kindly withdraw my pension money form my 1st
company MHBAN0128205000000017 ME-GWM.
