6. Brief Description of Incident Facts: (include name of IP(s) if possible)

Incident has been report by HSE Officer Mr. Abid Ali. The incident was reported at around 9:00am on 20 April
2024, Saturday happened at TWS4. While doing site inspection it was observed that 6 pieces of plastic
barriers was damaged due to hit by vehicle at night time. Mr. Abid Ali immediately informed the supervisor
and replaced the plastic barriers with the new one, refilled with water and soil immediately as shown in the
pictures below.

7. Brief Description of Action(s) Taken:

 Immediately informed the site supervisor to replace the damaged plastic barrier.
 Immediate stored and tied the damaged plastic barrier to secure from wind.
 Refilled all the plastic barriers with water and soil.
