Dear Dr. Banerjee,

RE: Ms. Eleanor Bennet, D.O.B.: 06/12/1975

I am writing to refer Ms. Eleanor Bennet, a 45-year-old commercial lawyer, who has been
experiencing side effects of captopril and is now reluctant to start new treatment, for your
expert advice and a review of her medication.

On being emergently admitted on 08/02/2021, Ms. Bennet was diagnosed with a heart attack
after a long-haul flight. Subsequently, she underwent a ballon-expandable stent. On
15/02/2021, she was discharged home with a 4-week off-work certificate and was commenced
on captopril 50mg twice a day and atorvastatin 80mg daily. Additionally, recommendations
about her lifestyle changes were given, and she was reffered to a cardiac rehabilitation;
however, she did not attended it.

Today, Ms. Bennet reported dizziness, light-headedness, headaches, and diarrhoea after taking
captopril; therefore, she requested a discontinuation of this medication.

Ms. Bennet is divorced and is stressed about her children, who are currently living with their
father. Please note, she drinks 40 units of alcohol weekly and smokes 29 cigarettes a day. Her
BMI is 29, and she exercises once monthly. Her father had a heart disease.

Given the above, it would be appreciated if you could review Ms. Bennet's medication and
manage her lifestyle.
