I, MOHAMED MIHILAR MOHAMED YASEER, father of YASHFA, hereby provide my full consent and NO
OBJECTION for my daughter to travel to Bangladesh with her grandparents, MOHAMED RAJAB
MOHAMED BASHEER (Passport No: N5618775) and FATHIMA PIYASA MOHAMED RAJAB (Passport No:
N5618773).

YASHFA was born on 13/07/2016 and holds a valid passport (Passport Number: N11374190). She will be
traveling with her grandparents from 04 JUN to 10 JUN, and they will be staying at NEW GRAND PARK
HOTEL, Bangladesh.

I confirm that I have no objections to this travel arrangement and fully trust MOHAMED RAJAB MOHAMED
BASHEER and FATHIMA PIYASA MOHAMED RAJAB to take care of my daughter during this trip. They have
my complete authorization to make decisions regarding YASHFA’s health, safety, and general well-being
while they are in Bangladesh.

Attached to this letter are copies of:

1. My identification (passport copies of both Father & Mother), Proof of Marriage certificate

2. Child Birth Certificate copy

3. The grandparents' identification (passports)

4. The grandparents' travel itinerary
