Maruz Subdivision, Brgy. San Isidro, General Santos City, South Cotabato, Philippines
9500 after having been sworn in accordance with law, hereby depose and state that:

1. I am the father of the visa applicant, Gherneil Belgira Dalanon, born on April
08, 2002, in Mindanao Medical Center, General Santos City, under Philippine
passport number P65068746, valid until Feb. 18, 2034;
2. He will be traveling with the Good Christian Ensemble to Preveza, Greece to join
in the 41st INTERNATIONAL CHORAL FESTIVAL of PREVEZA and 27th
INTERNATIONAL CHORAL COMPETITION and tour on June 27 to July 9, 2024;
3. As his father, I hereby accept and assume any and all responsibility for his
welfare related to his travel and stay in Greece;
4. I guarantee that he will abide by all laws and regulations of governments of
Greece;
5. This affidavit is for the purpose of attesting to the truth of the foregoing facts,
in support of my son’s application for Schengen visa.
