Dear Ms. Quejada and Mr. Casimiro,

I am writing to request your kind consideration in allowing me to obtain an index card today,
which I was unable to acquire during last week's orientation due to a work commitment. As I
have an upcoming exam scheduled for July, having an index card would greatly assist me in
adequately preparing for the test.

I understand the protocols and procedures in place regarding the distribution of this
requirement and I sincerely apologize for any inconvenience in my early out during the
orientation may have caused. I have attendance on that day but I can't get the index card with
stamp. I would be immensely grateful if you could accommodate my request for an index card
today. Your understanding and support in this matter would be truly appreciated.
