TECHNIQUE
Ø AP view
- BN nằm ngửa
- Chân xoay trong 15 độ (không áp dụng trong chấn
thương)
- Tia trung tâm:
+ 5 cm trên khớp mu
+ hoặc giao điểm đường nối hai gai chậu trước trên và
khớp mu

- Tiêu chuẩn: quan sát được
+ Toàn bộ khung chậu bao gồm cả L5
+ Xương cùng cụt
+ Đầu trên xương đùi bao gồm cả mấu chuyển
lớn.
+ Chỏm xương đùi cần tương đồng về kích thước,
vị trí
APPROCHING TECHNIQUE
Proximal femur
Scaral foramina + the cortex of femoral head, neck, greater, and lesser
+ arcuate lines: mềm mại, đối xứng trochanter: mềm mại, bè xương bình thường.
Nếu gập góc à gãy x, cùng Nếu mất liên tục vỏ xương, bè xương hoặc dải đặc xương ngang
à gãy đầu gần x.đùi
+ Shenton line.

nhỏ không lan vào vùng tỳ đè rộng lan vào vùng tỳ đè

- Tỷ lệ AVN thấp (10%), biến chứng không
liền thấp à điều trị hạn chế mất vững và
biến dạng sau mổ.

Type 1: two-part fr không di lệch
Type 2: two-part fr có di lệch
Type 3: three-part fr kèm vỡ vụn thành sau ngoài
Type 4: three-part fr kèm vỡ vụn thành sau trong Type 1 Type 2
Type 5: Vỡ vụn (>4 phần) cả thành ngoài và trong
Variant: đường gãy chếch từ bờ trong mấu chuyển nhỏ ra phía dưới ngoài subtrochanteric cortex

- Thường gặp ở người già 4 5
- Tỷ lệ AVN thấp (10%), biến chứng không
liền thấp à điều trị hạn chế mất vững và
biến dạng sau mổ.

Type 1: two-part fr không di lệch
Type 2: two-part fr có di lệch
Type 3: three-part fr kèm vỡ vụn thành sau ngoài
Type 4: three-part fr kèm vỡ vụn thành sau trong Type 4 Type 5
Type 5: Vỡ vụn (>4 phần) cả thành ngoài và trong
Variant: đường gãy chếch từ bờ trong mấu chuyển nhỏ ra phía dưới ngoài subtrochanteric cortex

- Tỷ lệ chậm liền cao hơn (46% so với 1%)
