2024 SLV QUILT GUILD SHOW ENTRY FORM

Quilt Show is scheduled July 1 – July 22, 2024 at the Alamosa Public Library, Alamosa, Colorado.
Entry forms may be given or mailed to Janet Davis, 328 Lakewood Drive, Alamosa, CO 81101.
Quilts must be clean; ready to hang with a 4” sleeve attached to the back; lower half of this form attached to the
back.
Enter small quilts 30” x 60” maximum size. In addition to the small quilts 5 large quilts will be displayed. The
Committee will determine which quilts will be displayed.
Submit the top of this form to the Quilt Show Committee by June 15, 2024.

hold harmless the SLV Quilt Guild, all members thereof, and the Alamosa Public Library and their
employees for loss, theft, or damage to my quilt(s). If this quilt is for sale, my contact information may
be given to an interested buyer.
