AFFIDAVIT OF LOSS

I, JOYLYN B. DEGALA, of legal age, single, Filipino, and a resident of Brgy. San Pedro 2
Malvar, Batangas, after having been sworn to under law hereby depose and say that:

1. I am a holder of a License to Exercise Security Profession with the license ID NR. R06-
2023210000-436 issued by the Philippine National Police in Camp Crame, Quezon City;

2. Sometime on April 19, 2023, in the afternoon, I lost the above License to Exercise
Security Profession under the following circumstances;

I lost the said ID when I put it in my wallet and when the moment I needed the said
ID I could find it and I think I lost it unintentionally in my workplace.

3. Despite diligent efforts exerted, I could no longer locate, find, or recover the said
License to Exercise Security Profession, and the best of my knowledge, the same is truly
lost.

4. The said License to Exercise Security Profession with a license has not been confiscated
by any officer of the law or the issuing office or agency due to the violation of any law,
statute, order, rule, or regulation.

5. I execute this affidavit to attest to the truthfulness of the foregoing information, and to
secure another License to Exercise Security Profession, and for whatever legal purpose it
may serve.
