Guide on how to answer the Student Feedback Form
SY 2023-2024
Why is student feedback important?

The feedback that we


will gather helps the
school, departments,
and teachers to reflect
and improve towards
more effective and
engaging learning
experiences for
students.
How is student feedback gathered?
Every end of term, students are given a
Google Form link to the Student Feedback
Form (SFF) where they are asked to
evaluate their subject teachers.

The form has 3 parts:

1. Student and class details


2. Teacher behaviors to be rated
3. Open-ended questions

1. My teacher responds to my questions during class hours.
2. My teacher provides clear and complete answers to my
questions, which help me understand and finish my
tasks, modules, and outputs.
3. My teacher helps me understand the lesson and what I
was expected to learn.
4. My teacher uses strategies that encourage me to
participate in class activities, whether individually or in
groups. This is how each item looks
5. My teacher treats students with respect (polite tone and like in the Google Form.

a 7-point scale

The last part of the form gives an


opportunity to elaborate on learning
experiences by sharing what
activities helped or could help
students learn better. There is also
a question about learning modules.
