COMPANY EQUIPMENT ACCEPTANCE & PLEDGE

This is to acknowledge that I have received the company equipment stated below in trust to allow me to perform
my job function more efficiently and effectively.

THE RESPONSIBILITY OF THIS EQUIPMENT RESTS SOLELY ON ME.

IT IS AGREED AND UNDERSTOOD THAT:

1. I shall take care of this piece of company equipment entrusted to me, as if it is my own personal effects
with the greatest care and attention.
2. I understand that the equipment belongs to the company and shall be returned together with all its
hardware, accessories, and software, as well as any password and IDs, at any time upon request of the
management.

I SHALL NOT DO THE FOLLOWING:

1. To leave this equipment unattended in an unsafe or unsecured environment.

2. To subject the equipment and its accessories to harsh, or rough treatment.
3. To lend or allow the equipment to be used by any unauthorized persons.
4. To leave the equipment in my vehicle or other places where risk of theft or loss is high even for a moment
or in another similar environment.
5. To leave the equipment to be exposed to high risks damage environment.
6. To install any software/password locks or modifications that may damage or jeopardize the integrity of
the company information contained in it.
7. To upgrade or fix or otherwise modify the equipment without prior permission.

If any of the above is found to be true and happen as a result of my own negligence, I may be responsible to
personally replace or repair the equipment entrusted to me at my own expense or other compensation to the
company in lieu.
