AFFIDAVIT OF
. DISCREPANCY

I, JAYSON CASTILLO BANASTON, of legal age, single, with residence

at #92 Southgate, Barangay Pansol, Quezon City, after being sworn in
accordance with law, do hereby depose and state that:

1. I was born on 10 April 1991 with a given name JAYSON CASTILLO and
was acknowledged by my biological father as his son;

[A photocopy of my Certificate of Live Birth issued by the National Statistics

Office (now named as Philippine Statistics Authority is attached as an integral
part of this affidavit as Annex “A”]

2. As the law existing at the time of my birth prohibited me from using the
surname of my father, my parents were constrained to register such name before
the Local Civil Registrar;

3. At the time I was enrolled in school, my parents wanted me to use the

surname of my father to avoid any issues with regard to my name, hence, from
then on, I have used the name JAYSON CASTILLO BANASTON;

4. In all my public documents, school records and other public dealings, I

have used the name Jayson Castillo Banaston, including my application for
government issued Identification Cards and other papers in support of my
present employment;

[Photocopies of my TIN Identification Card and COMELEC issued Voter’s

Identification Card are collectively attached in this affidavit as Annex “B”]

5. To clarify this discrepancy, I hereby declare that the person named

“JAYSON CASTILLO” as appearing in the certificate of live birth issued by the
National Statistics Office, the person named “JAYSON CASTILLO BANASTON”
as appearing in my TIN Identification Card and COMELEC issued Voter’s ID
ARE ONE AND THE SAME PERSON;

6. I am executing this Affidavit to attest to the truth of the foregoing statement

and for whatever legal purposes it may serve.
