Field Placement and Seminar

The following checked item(s) have been determined by faculty as areas that require immediate
attention to promote your success in Field Placement and Seminar. Please review and sign the
agreement with the outlined conditions.

The Learning Contract is in effect once emailed by faculty to your SLC account. It is your
responsibility to monitor your emails.

1) Ask for feedback from Faculty, FP Supervisor and colleagues on a regular basis at
placement to ensure expectations are being met

2) Report any difficulties at placement to your FP Supervisor and Faculty to assist in

resolving these issues in an efficient manner

3) Always contact both the FP Supervisor and your Faculty in advance (prior to the start of
the shift) if absent or late from placement

4) Maintain positive active participation in your FP and seminar course

5) Plan, prepare, and review your logged hours to address any gaps or challenges

6) Abide by the OACYC and Student Code of Conduct at all times

7) Maintain professional boundaries at all times with staff and clients

8) Abide by the assigned roles and responsibilities of the student within field placement

9) Abide by all field placement expectations and competency requirements as outlined in the
CYC FP Guide

Not following through on agreed conditions could result in immediate termination and failure from field
placement and seminar. This learning contract will remain in effect for the remainder of your field
placement unless otherwise identified.
