AFFIDAVIT OF NO PENDING CIVIL, CRIMINAL

OR ADMINISTRATIVE CASE

I, ROSE-ANNE GALIDO MABASA, of legal age, single, Filipino, and a resident of

Purok 7, Barangay Calaocan, Bambang, Nueva Vizcaya, Philippines, after having been
duly sworn to an oath in accordance with law, hereby depose and state that:

1. I am currently an elected Sangguniang Kabataan Kagawad of Barangay

Calaocan, Bambang, Nueva Vizcaya;

2. I have no pending case, criminal, civil or administrative, before any judicial

authorities or agency in the government;

3. I am neither an advocate nor a mentor of any unlawful or subversive

organization that advocate he overthrow of the government or agency in the
government;

4. I was never accused of any offense or crime involving moral turpitude, neither
one of the same nature or a pending case of anti-graft law (R.A 3019) is
pending before any prosecution office and judicial authorities elsewhere in
the Philippines;

5. I am executing this affidavit to attest the truth of all the foregoing declarations
and for whatever legal purpose this may serve.
