TRAINING UNDERTAKING

I, <NAME>__________, do hereby undertake and agree to complete the

program under <TRAINING TITLE>__________ , on
<TRAINING DATE> ____.

It is understood that by signing this TRAINING UNDERTAKING, I hereby agree to:

1. Fulfill the required service obligation of six (6) months which shall reckon from the
completion date of the above Training Course or in the event of existing
training/scholarship grant service obligation, the service obligation for this activity
shall start upon completion of the former;

2. Refund the amount equivalent to the entire course fees, incidental expenses and
other related fees paid by the PS-DBM to the provider of the course for failure to
render in full the service obligation herein stipulated on account of my voluntary
resignation, optional retirement, separation from the service through my own fault,
or other causes within my control;

3. Authorize the PS-DBM to deduct from my salary or any monetary benefits that I am
entitled in the event of my failure to render the service obligation;

4. Perform the duties and responsibilities appurtenant to my employment with the PS-
DBM during the Training;

5. Reimburse all the fees paid by the BENEFACTOR should I drop out from any or all
course during the Training; and

6. Strictly comply with the terms and conditions of this service contract.
