EVENT PARTICIPATION WAIVER

ISABELA EVENTS SUPPLIERS BASKETBALL LEAGUE

I, ______________________________________, hereby acknowledge and agree to

participate in the Isabela Events Suppliers Basketball League scheduled to take place on
March 05, 2024, at the Cabatuan Sports Stadium, Cabatuan Isabela. In consideration for
being permitted to participate in this event, I hereby voluntarily assume any and all risks,
both known and unknown, associated with my participation.

I understand and acknowledge that basketball is a physical sport that involves inherent risks,
including but not limited to bodily injury, strains, sprains, fractures, and even serious
accidents. I acknowledge that the organizers, sponsors, and other participants of this event
cannot guarantee my safety and well-being during the course of the game.

In consideration for being allowed to participate, I hereby waive, release, and discharge all
claims for damages, injuries, or losses against the Isabela Events Suppliers, its organizers,
sponsors, employees, agents, and volunteers arising out of or connected with my participation
in this basketball event. This waiver includes, but is not limited to, any claims for negligence,
premises liability, or breach of warranty.

I further agree to indemnify and hold harmless the Isabela Events Suppliers, its organizers,
sponsors, employees, agents, and volunteers from any and all liabilities, claims, actions,
damages, costs, or expenses, including attorney fees, that may arise as a result of my
participation in the basketball game event.

I certify that I am physically fit and able to participate in this event, and I acknowledge that I
have been advised to consult with a physician before engaging in any physical activity if I
have any concerns about my health or ability to participate.

I understand that this waiver and release shall be binding on me, my heirs, executors,
administrators, and assigns, and that I am giving up substantial rights by signing it. I have
read and understood this waiver in its entirety, and I voluntarily agree to its terms.
