Practical Placement Agreement

VOCATIONAL PLACEMENT AGREEMENT


We agree to the Vocational Placement under the CHC33021 Certificate III in Individual Support training
program, on the following terms, covenants, and conditions:
1. The provisions of the following industrial relations and legislation requirements are adopted and
incorporated into this Agreement and operate as terms, covenants and conditions of this
Agreement:
 The current Fair Work Act
2. The duration of Vocational Placement must be a minimum of 120 hours.
3. The period of this Agreement is two years from the date agreed to in this agreement. To be
clear, this Agreement expires two years from the date of execution of this agreement.
4. A student on work experience is not an employee of the work experience provider, and the
provider is not the employer of the student.
5. All information regarding the RTO, student and vocational placement provider is confidential and
must not be discussed with a third party.
6. All parties have reviewed and agree to the training plan.
7. Laurels Education and Training warrants that it has:
a) Public liability insurance policy covering this vocational placement arrangement.
b) Satisfied itself that the vocational placement can provide, or arrange to provide to the
student, the facilities, range of work, supervision and training required under the training
plan.

RESPONSIBILITIES OF THE REGISTERED TRAINING ORGANISATION


In order to ensure that appropriate care is exercised, the Registered Training Organisation (Laurels
Education and Training) has a number of responsibilities to both the placement organisation and the
student. As the Registered Training Organisation, Laurels Education and Training is required to:

 Ensure that the placement organisation and the student are aware of their rights and
responsibilities and the student is prepared for the workplace in accordance with the course
requirements.
 Ensure that all students have a current Police Check prior to commencement of the
placement in a care facility – copy to be forwarded to the host organisation
 Provide support and guidance to the student throughout the placement as set out in the
Vocational Placement Information Package
 Liaise and mediate between the placement organisation and the student to resolve any
issues that may arise
 Monitor progress and inform both the placement organisation and the student of any
changes
 Provide the student and the placement organisation with an overview of what is expected on
placement to meet the requirements of the qualification
 Confirms that the vocational placement organisation can provide access to resources
required during vocational placement outlined in the responsibilities section of the
Vocational Placement Provider/Supervisor Information Pack,

RESPONSIBILITIES OF THE VOCATIONAL PLACEMENT ORGANISATION


In order to ensure that appropriate care and supervision is exercised, the vocational placement
organisation has a number of responsibilities to, and in respect of a student placed under this
arrangement. As the vocational placement organisation you need to:

 Agree to act in accordance with the workplace responsibilities, including those set out in the
relevant Work Health and Safety Act and Equal Opportunity legislation.
 Provide adequate levels of supervision to ensure the welfare and safety of the student in a
non-discriminatory and harassment free working environment
 Provide supervision and instruction to the student for the purposes of the placement and
provide opportunities to complete tasks and activities set out in the Skills Journal and verify
said tasks as they are completed by the student.
 As the placement organisation, provide on the job training and supervision
 Ensure the student is not left unsupervised with residents/clients at any time
 Permit access to a representative from Laurels Education and Training, as Registered
Training Organisation, at any reasonable time. Times are to be negotiated with all parties.
 Nominate a supervisor of the student who will be responsible for carrying out the placement
organisation’s obligations
 Confirms that the nominated supervisor has read the Vocational Placement
Provider/Supervisor Information Pack and will fulfil the Vocational Placement responsibilities
outlined within.
 Acknowledge that the student is not a worker within the meaning of the current Fair Work
Act.

RESPONSIBILITIES OF THE STUDENT


In order to ensure that the practical placement is a beneficial learning experience the student has a
number of responsibilities under this arrangement. As the student you are required to:
 Confirm that you have read and understand the Vocational Placement Student Information
Pack and are aware of your rights and responsibilities.
 Perform placement duties and complete tasks and activities set out in the Skills Journal to
the best of your ability and comply with all reasonable directions of the vocational placement
provider and its employees.
 Agree that this does not constitute an employment relationship with the employer
 Comply with all reasonable workplace rules and requirements relevant to safety and
behaviour.
 Notify the supervisor at the placement organisation and your Trainer from Laurels Education
and Training of any accidents, injury or incident that may occur
 Ensure punctuality and attend the placement organisation according to the roster
 Notify the placement organisation and Laurels Education and Training if unable to attend
 Ensure the dress code of the placement organisation is adhered to
 Work in a safe manner and use all protective clothing and equipment as required
 Treat all people with respect and free from discrimination
 Advise if there are any known medical factors, including medication or treatment, that may
impact your ability to perform duties safely during placement.
 Consent to receiving such surgical or medical treatment (including the administration of an
anaesthetic) as may be deemed necessary by a qualified medical practitioner should you
become ill or injured while on placement.
 Arrange your own transport to and from the workplace.

Practical Placement Agreement

INSURANCE COVER
Students undertaking practical placement during training with Laurels Education and Training (LET)
may be eligible to make a claim under the ‘workers’ compensation’ insurance policy held by the
Department of Education and Training.
If a student is injured while undertaking practical placement and wishes to make a claim for
compensation the following process should occur: -
 The student should complete and sign a Workers’ Injury Claim Form and an Employer Injury
Claim Report Form (both forms), in consultation with the employer and/or LET, if necessary.
 LET should complete and sign the employer section of the Workers’ Injury Claim Form and
Employer Injury Claim Report Form (both forms), in consultation with the employer if necessary
 LET should clearly mark on both forms ‘VRQA practical placement claim’ and include the
Employer Scheme Registration number of 1624618 and the employer’s reference number of
9573347 on both forms.
 LET will scan and email the Employer Injury Claim Report and the Worker Injury Claim forms
along with any certificates of capacity, completed incident notification form, medical accounts
and a certified copy of the practical placement agreement to Gallagher Bassett at;
educlaims@gbtpa.com.au.

PUBLIC LIABILITY and PERSONAL ACCIDENT


The student is covered by Laurels Education and Training’s public liability and personal accident
insurance coverage whilst on practical placement. This agreement must be signed by all parties and
returned to Laurels Education and Training for signing to ensure insurance coverage.
