JOINT AFFIDAVIT OF TWO (2)

DISINTERESTED PERSONS

We, BENJAMIN M. BALANON and MARIA L. COYUPAN, both senior

citizens, Filipino citizens and residents of Guinzadan Norte, Bauko, Mt. Province,
under oath, depose and state, viz-

1. We personally know EDUARDO EDDIE T. CADWISING as he is our long

time family friend and neighbor in Guinzadan Norte, Bauko, Mt. Province;

2. We are aware that EDUARDO EDDIE T. CADWISING had been using the
names “Eduardo” and “Eddie” in some of his personal documents and
identification cards;

3. The use of nicknames or single names had been common upon us. Such
became our practice when we were in the elementary grades as we were then
having difficulties writing our complete names specially if our two first names are
long;

4. The acts of EDUARDO EDDIE T. CADWISING in using the names

“Eduardo” and “Eddie” are solely due to the above reason and not for purposes
of committing fraud;

5. We are executing this affidavit to attest to the truth of the foregoing and for
any legal intents and purposes that this Joint Affidavit may serve.
