AFFIDAVIT

I, WALTER B. KILONGAN, of legal age, married, Filipino citizen and a resident of Upper
Brookside, Baguio City, under oath, depose and state, viz –

1. I am the authorized driver of a 1996 Isuzu Pick-up with plate number UNH 801 which
is owned and registered under the name of Jerry E. Sagayo;

2. On October 6, 2010 at around 4:30 o’clock in the afternoon, I went to Easter College
located in Central Guisad, Baguio City to pick-up the kids of Jerry E. Sagayo;

3. I parked the above cited vehicle at the designated parking area of Easter College
which is located below the school. Subsequently, I got out of the vehicle to look for the
kids of Jerry E. Sagayo;

4. After picking up the kids, we then proceeded at the parking area;

5. When we were at the parking area, I noticed that the front portion of the Isuzu Pick-
up was badly hit. The car’s front portion was totally damaged;

6. I asked the neighboring parked vehicle drivers and the assigned security guard at the
area if they had seen or noticed the bumping incident but all of them answered in the
negative;

7. I immediately went to the nearest police sub-station to report that the Isuzu Pick-up
was heavily damaged due to a “hit and run” incident.

JOINT AFFIDAVIT
We, RAMON J. JOSE and JOEL SAMATRA, married and single respectively,
Filipinos, and residents of Baguio City, under oath, depose and state, viz-

1. We are the respective Branch Manager and cashier of DELTA MOTOR SALES
CORPORATION;

2. On May 20, 2010, at around 3:20 o’clock in the afternoon, an ISUZU TROOPER with
plate no. RFA 540, was parked inside our service station for repair and maintenance;

3. While the said ISUZU TROOPER was jacked in our “power lifter”, it accidentally
slipped and fell which caused the vehicle the following damages:

a. Dent on the right front door;

b. broken window glass; and

 c. broken rain gutter.

4. The above vehicle is registered at the Land Transportation Office in the name of
Tessia A. Panta while the repair and maintenance account at DELTA MOTORS is
registered in the account of Swanny Mangaoang;

5. We are executing this affidavit to attest to the truthfulness of the above narration of
facts and for all legal intents and purposes which this affidavit may serve.

Affiant further says none.
