Regarding Mr Simon Ham
TAC claim number 23/92377

I am writing to you about Simon’s TAC claim in relation to a transport accident on 12 October
2023.
TAC is seeking clarification around the accident circumstances and requires further
information by the way of interview and written statement from your client.
Please provide a signed statement from your client regarding the accident using the
following questions:
Interview and obtain a statement from the client and establish:
1. Ascertain the location of the bike on the road when the accident occurred
2. Obtain full description of the incident and what caused the fall from the bike?
3. Where was the client going at the time of the incident?
4. Where had the client been just before the incident?
5. If an unknown vehicle was involved, can they describe the colour, make and
model of the vehicle and provide a description of the driver?
6. Establish where on the road way the client was riding immediately prior to
the incident, if an unknown vehicle involved, which direction was the vehicle
coming from, where was the unidentified vehicle when the client first saw it?
7. When did the client first see the vehicle? Approximate distance in metres
8. What speed was the vehicle travelling at?
9. Did the vehicle stop and render assistance? If the driver provided name an
contact details please provide
10. How was the client transported from the scene? Identify the person and
interview.
11. Was the bike damaged as a result of the accident?
 12. Provide details of the model of bike the client was riding at the time of the
accident
13. Was the bike repaired? If yes, who made the repairs? Identify and interview
the repairer, provide copy of quotes for repairs, invoices for repairs, any
photos of the damage etc.
14. If no repairs have been made to the client’s bike, provide photos of the bike.
15. If the road way had tram tracks, how far from the tram tracks did client fall
from their bike?
16. Were there any witnesses to the accident? Please provide name and contact
details
17. Was the vehicle on the right side of the road?
18. Was the client employed at the time of accident? (please provide
employment details)
19. What did the client do with their bike following the accident? If client left bike
somewhere, please establish where for enquiries to be made.
20. Did the client contact anyone from the scene / provide phone
records/messages sent etc. to support this.
21. Who was the first person the client saw following the accident? Provide
name and contact details
