I’m Maria Chestnutwood, Integrated Claims Examiner and I am assisting you during your
leave of absence to care for your parent, Christie Donovan under the Family and Medical
Leave Act (FMLA) and applicable state laws.

It is your responsibility to send me completed documentation by 04/21/2024. You can fax it
to me at 866-683-9548, take a picture and upload it using the mobile app or mail it to Matrix
Absence Management, Inc.
If the documentation is not received by 04/21/2024 or if sufficient information is not provided
in a timely manner, your leave may be denied.
Any time off that is not approved may be applied to Banner Health’s attendance policy and
may result in disciplinary action up to and including termination.
