Bukidnon, after being duly sworn to in accordance with law, depose and

1. We are the surviving heirs of Orencio L. Pensahan who died on August 5,

2020 in Sumpong, Malaybalay City, Bukidnon;

2. That Orencio L. Pensahan was survived by his wife and the following

no other heir or heirs except for the above-named heirs of the said deceased
