Brief description of the incident: while starting wagon loading
procedure from stack 201, 6 wagons were pulled from dead-
end side of shipping yard towards stack 201 using crane 15.
During pulling of wagons, at track junction/changeover point
the wagons got derailed from their track. Later traffic
department was called to rerail it in track. No material or
equipment got damaged.

Probable cause of incident: (Unsafe Condition/Unsafe Act):


The matter was investigated and found that the near miss
happened due to not changing the changeover lever from
side line towards straight line/road side of shipping yard
before starting any wagon movement across the junction,
which is an unsafe act as it can lead to damage of cranes
ropes and even falling of wagons.
Corrective action: To avoid such incidents in future following
steps were taken by different agencies:
Operation: Operators were advised not to pull wagons with
cranes other than in case of extreme exigencies that too
with extreme caution, also supervisors in yard were given
strict instructions to always check track junction lever
position before and after loading of all wagons.
