Brief description of the incident: During wagon loading of plain
rounds from stack 103 at 21:30 hrs the CRD cable
transmission box for the magnets got displaced from its
frame and got tilted. As per the crane operator- P Srinivas
Rao (B-2) and nearby loading personal one magnet of the
crane got stuck in the inspection window of the wagon and
the wagon got lifted a bit, though the operator skillfully took
the magnet out of the window, in attempt of taking it out
the bolts/welding of the transmission box got sheared, but
the wagon was saved from getting derailed from its track.
The Incident was immediately reported to then cranes
electrical Shift In charge - Shubham (DM)

Probable cause of incident: (Unsafe Condition/Unsafe Act):

The matter was investigated and found that the near miss
happened due to magnets getting stuck in wagon inspection
windows, which is an unsafe condition as it can lead to
damage of various equipments of cranes as well as can cause
derailment of wagons.
Corrective action: To avoid such incidents in future following
steps were taken by different agencies:
Operation: operation advised the Traffic department to
develop grilled type inspection windows which eliminates
the stucking of magnets.
 Electrical: started periodical check list for transmission box,
its health and robustness.
