APPLICATION FOR PROBATION and

MANIFESTATION

COMES NOW the accused AAA, through the undersigned

counsel, unto this Honorable Court most respectfully states:

1. On November 17, 2016 the accused received a copy of the

Honorable Court’s Judgment on even date, finding him GUILTY of
the crime of Direct Assault Upon an Agent of a Person in Authority
(Criminal Case No. 04065) and sentencing him to an
indeterminate sentence of four (4) months and one (1) day of
arresto mayor as minimum and three (3) years, six (6) months
and twenty-one (21) days of prision correccional as maximum ,
and to pay a fine of One Thousand Pesos (1,000.00);

2. Accused is likewise found GUILTY of the crime of violation of City

Ordinance No. 5121 (Criminal Case No. 04066) and sentencing
him to pay a fine of Two Hundred (P200.00).

 3. That the accused has not been previously convicted by final
judgment of any crime or offense;

4. That the crime for which the accused has been convicted is not an
offense against the security of the State;

5. That the accused has not previously undergone probation under

PD 968, as amended;

6. That the accused has all the qualifications and none of the
disqualifications under P.D. 968, as amended.

7. Accused likewise manifests that he has already paid the fines

imposed in the above entitled case. Attached hereto are copies of
Official Receipts Nos. __________for the fines imposed in
Criminal Case Nos.______, respectively.

PRAYER
WHEREFORE, premises considered, it is respectfully prayed of this
Honorable Court that this application for probation be given due
course.

Other reliefs just and equitable under the premises, are likewise
prayed for.
