I, MICA ELLA ALOQUEN CORNEL, of legal age, Filipino, and a resident of A-1
Sitio Libjo Sto. Niño, Parañaque City, after having been sworn to in accordance with law,
hereby depose and say that;

1. That I am the mother of CHENG TIAN CORNEL OUYANG who is the affiant
herein with his birth details being born on May 14, 2022 in Parañaque City,
Philippines;

2. That his name is CHENG TIAN CORNEL OUYANG as stated and registered in his
birth certificate issued by the Local Civil Registrar in Parañaque City, Philippines;

3. That I hereby certify and affirm that “JIA LIN OUYANG” as appearing in his Child
Immunization Record and “CHENG TIAN CORNEL OUYANG” as appearing in
his birth certificate, refers to one and the same person;

4. That I am executing this affidavit to attest to the truth of the foregoing facts and
whatsoever this may serve
