Revenue Service concerning Income made on Bonds issued by your self and your Court in the
name of PEACE LOVE LIGHTNING,associated with SOCIAL SECURITY TRUST ACCOUNT #
415-29-8881. PEACE LOVE LIGHTNING is reporting to the IRS and the amounts of the Bonds
are necessary for the IRS to appropriate the correct amount of taxation on the Original Issue
Income for the abovementioned SOCIAL SECURITY number.

PEACE LOVE LIGHTNING TRUST requests the completed W-9, as well as the total
number of Bonds and the amounts of Each that were issued in the name PEACE LOVE
