PROCEDURE ON PERFORMANCE OF

NATIONAL EXTERNAL QUALITY ASSESSMENT SCHEME (NEQAS)

As per DOH Memorandum 2009 - 0086B, the following are the National Reference Laboratories (NRL), parameters,
and frequency of NEQAS.

PROCEDURES
1) Download necessary forms from the website of each NRL/NEQAS provider.
2) Completely fill-out the forms.
3) Pay according to the instructions provided by the NEQAS provider.
4) The NEQAS materials will be received weeks after completing the registration.
5) Upon receipt, check sample condition and integrity. Check transport temperature. Check for leakage.
6) Do not accept damaged samples.
7) If acceptable, store at proper storage temperature.
8) Once schedule of running arrives, before running, perform daily quality control (QC).
9) Run NEQAS samples the same way as patient samples.
10) Final results are input in the prescribed website for NEQAS submission by the NEQAS provider. Results are
submitted on or before deadline of submission.
11) Weeks later, a certificate of participation wilsl be sent by the NEQAS provider.
12) If results passed the national standards, a certificate of proficiency will be given.
