FOS TREZBIEN DRONE INCIDENT

(21JUL23)

I. HOLDING STATEMENT: An unmanned aerial system assigned to the U.S.


Army's 2nd Brigade Combat Team, 1st Cavalry Division supporting the 4th Infantry
Division lost signal and crashed at approximately 6pm on July 20th, 2023 near
Trezbien, Poland. There was no damage to property or local infrastructure. The
incident remains under investigation.

II. BACKGROUND INFORMATION:


BLUF: on or about 20 1800 JUL 23 a Shadow lost signal and crashed. No
injuries. Initially assessed as Class B. The accident occurred roughly 2000m
East of FOS Trezbien. There was no damage to local infrastructure and minimal
local national presence. We are on site securing and identified
field grade IO is en-route. This is an initial report.

WHO: Operators: SPC Catona, SGT Martin, SPC Vandorp

WHAT: Class B Aviation accident - UAS AV 3356

WHEN: Notified 1800 20JUL2023

WHERE: 2000m east of FOS Trzebien

SUMMARY: AV 3356 experienced a servo on the wing flap. The operators


began their landing procedures when the UAV lost its signal link to the Ground
Control Station (GCS). The UAV automatically flipped upside down to deploy
parachute and save its payload. No human injuries are assessed. Three local
national policemen are on ground to ensure safety of citizen and soldiers.
