The trainee agrees to serve the attachment provider during the stipulated period to learn the trade and the
attachment provider will facilitate suitable duties.

1. Attend to all duties assigned

2. Obey the rules and regulations in the organization without exception

3. Take care of any property under his/her care

4. In case of need to be absent, get permission from the supervisor

5. Not divulge any classified information to outsiders

6. Not engage in any form of personal business during work hours

7. Avail himself/herself for assessment by authorized person as advised by the Industrial Liaison Officer
or supervisor

8. Co-operate with fellow workers

9. Take of Insurance cover

10. Make a written report on attachment as may be required at the end of the attachment period.

1. Induct and orient the trainee on the organization

2. Place the trainee in diverse tasks that give maximum use of training skills and abilities

3. Place the trainee under a supervisor

4. Endorse logbook on daily basis

5. Ensure security and protection to the trainee while in the course of duty as other employees

6. Liaise with the industrial attachment office on matters of personal conduct of trainee.

7. Make a confidential report on the trainee on the form to be provided by an assessor from the
polytechnic. (Let us collaborate to make the trainee a better employee in the future).

1. Complete all the details required in page 1 and 2.

2. Maintain the logbook every day of the duty by completing the information as required and submitting
to the supervisor at the end of each day endorsement.

3. The log book must be presented to the supervisor or external assessor on request.
4. Contact the industrial liaison officer in case of challenge. The polytechnic shall not be liable for any
money lost by trainee. Please avoid assigning duties involving handling money.

5. Make a report of the attachment using the guide provided in the last pages of the logbook.

6. The logbook must be kept clean and returned to the industrial attachment officer at the end of the
attachment period for further evaluation and provision of certificate.

7. In case of injury while on duty in the industrial attachment the trainee should:

(I). Report the injury to the industrial supervisor/management immediately.

(II). Seek medical attention and keep the treatment records clean and safe.

(III). Communicate about the injury to the institution/ILO within 24 hours
