CHECK LIST AND DOCUMENTATION

while submitting Composite Claim Form in Death Cases


Form-20 (PF Payment), Form-10D (Pension), Form-5IF (EDLI)

In order to minimise rejection and hassle free processing of claims pertaining to death
cases please go through the check list and documentation as mentioned below before
forwarding the claims to this office.

Please ensure all documents must be signed by Employer/Authorised signatory of the


establishment and claimant.

The following documents DULY ATTESTED are required for processing claims
pertaining to death cases are as under:

1. Death Certificate.
2. Copy of Nomination, if nomination has already been filed by member.
3. Surviving Family Member Certificate issued by Employer (SFMC).
 In SFMC Father & Mother status must be mentioned.
 And in case of female member her Father-in-Law and Mother-in-Law status
4. Legal Heir Certificate, in case where Nomination and SFMC is not available.
5. Passport size photographs of each claimant for pension (3 copies of each) duly attested
and identified by employer/Authorised signatory of the establishment.
6. Aadhaar of all eligible claimants.
7. Birth Certificate of Childrens.
 Date of Birth in Aadhaar and Birth Certificate must be same.
8. Bank Passbook of all claimants must be opened in same Branch of either in SBI or BOI
for payment of pension.
 For PF & EDLI in any Scheduled Bank
9. Separate Claim applications must be submitted duly signed by each eligible claimant.
10. Employer must update Date of Leaving and Reason of Leaving in system in
Exit Column of Employer Portal before forwarding the claims to this office.
11. In case of Death while in Service, please ensure contribution has been paid upto the
date of Death if not reason thereof with documentary evidence be submitted alongwith
the claims.
12. Left Hand Finger Impression in case of illiterate claimant, it must be identified by
employer/Authorised signatory of the establishment.
13. The employer/Authorised signatory of the establishment must check his validity of
DSC before signing and forwarding the claims to this office.

PF, EDLI & PENSION eligibility in case where Family of deceased member exit as per the Act Provisions.
