APPLICATION FOR POLICE CLEARANCE CERTIFICATE

After completing form 137 and the Finger-Print form (have your fingerprints
taken at your local police station), send your documents to the Embassy of
Zimbabwe, 1608 New Hampshire Ave. NW, Washington, D.C. 20009, with
a fee of US$40.00 in money order and a self-addressed, stamped envelope
for the return of your documents. Processing time is 5 working days.

Upon receiving the processed documents from us:

1. Deposit US$75.00 into the Officer Commanding Criminal

Investigation Department, Standard Chartered Bank, Highlands

Attach the telegraphic slip to the form 137 and the Finger-Print form
