REQUEST FOR QUALIFICATIONS

The Regional School District #16 Building Committee of the towns of Beacon Falls and
Prospect, Connecticut invites Statements of Qualification for Architectural/Engineering services
from qualified firms, licensed in Connecticut, for Preliminary Design and Referendum work as
follows:

Perform site analysis for a new Grades PK-5 Elementary School and advise the building
committee as to site suitability. Proceed with the Preliminary Design Phase and prepare cost
estimate for a new Grades PK-5 Elementary school on Tallmage Hill parcel, and assist the
building committee in presenting this project to the two communities prior to a referendum that
will be required for acquisition of the land and to approve the full cost of the project.

Interested design firms should request an information packet from the Regional School District
#16 Board of Education Administrative Offices at 207 New Haven road, Prospect, CT 06712 to
obtain further information and the specific requirements for submitting Statements of
Qualifications. Please contact Hugh W. Potter via phone (203) 758-6671 to pick up the packet

Statements of Qualifications must be received in the Region #16 Business Office by 3:00 p.m. on
Monday, January 31, 2010 at the above address.

FORMAT REQUIRED FOR STATEMENT OF QUALIFICATIONS

In order to expedite the committee’s review of a firm’s statements of qualifications, please


provide the following information in the order listed below:

1.0 A Letter of Transmittal signed by a principal of the lead firm, not to exceed two (2)
pages, describing in narrative form your firm or team and its qualifications and why you
believe yours is the best firm for this project.

2.0 Statement of Qualification to include, but not necessarily limited to, the following:

2.1 Name and address of your firm. Also indicate who your contact person will be
for this project and provide a phone number, fax number and e-mail address that
the Committee may use to issue further information.

2.2 A brief history of your firm along with completed Federal GSA Forms 254 and
255:

 Provide a list of your Project Team Members, including any consultants.

 List the staff proposed to work on this project along with related
responsibilities and the amount of time they will devote to this project.
Provide resumes of key personnel that would be assigned to this project and a
brief description of projects similar to this in which the key personnel have
worked together as a team. Refer (2.3) below, if possible.

 If a team will be used, please provide an organization chart showing the


personnel of the Design Team and their proposed involvement in the project.
Please include the qualifications of all team members. Please indicate how
this design team will be managed.

2.3 List projects that your firm has been involved with similar challenges to this
school building project within the past five years (include projects involving new
site analysis and presentations that resulted in successful referendum votes).

 Indicate if any claims, disputes or arbitration proceedings have occurred on


any of these projects. If so, identify if they were between Owner/Architect or
Owner/Contractor and give the status of each.

2.4 List five (5) references, of similar projects described in 2.3 above giving:

 List of Project Team Members, consultants and staff involved;


 Size (project cost and square feet) and location of project; and
 Provide contact name and telephone numbers for the Owner and Contractor
(specific individuals) of each project.

2.5 List the last five (5) public school projects that you have completed. Demonstrate
the Design Team’s ability to develop and control project costs, quality and
schedule.

 Indicate the proposed budget and construction schedule;


 List the total cost of change orders not requested by the owner.
 List the awarded construction contract amount; and
 Provide contact name and telephone numbers for the Owner and Contractor
(specific individuals) of each project.

2.6 A one-page statement of the firm’s recent experience on public school design
philosophy and its integration into the community.

2.7 Please indicate the location where the design work for this project will be
accomplished.

2.8 Additional information, not included above, that the firm feels may be useful and
applicable to this project. Please limit your responses to three (3) pages.

BACKGROUND:

In January 2011, following enrollment studies in October and December 2010, the Regional
School District #16 Board of Education approved the following with respect to its proposed
Grades PK-5 Elementary School Project for 606 students:

1. Accepted and adopted educational specifications for this project (see attached)

2. Authorized the preparation of schematic drawings and outline specifications for this
project

3. Authorized and filed a grant application with the Connecticut State Department
of Education on Form ED049

4. Established a Building Committee to oversee the design and construction of this project

GENERAL SCOPE OF WORK:

The selected firm will be expected to:

1. Validate site analysis and confirm Tallmage Hill site which should include but not be
limited to traffic, local zoning, subsurface conditions, wetlands, water, sewage, proximity
to current site, field availability, etc. We expect the design professional to provide a list
of names of any other firms he or she would ask to assist with this effort.

2. Proceed with the Schematic Design Phase and prepare a cost estimate for a new Grades
PK-5 Elementary School for 600 students on a new site.

3. Assist the building committee in presenting this project to the two communities prior to a
referendum that will be required for acquisition of the land and to approve the full cost of
the project.

TIMING

1. The committee anticipates hiring an architect on or about April 4, 2011and have the
school ready for occupancy on November 28, 2015.

FUNDING:

As a regional school system, Region #16’s funding comes primarily from our two communities.
The current budget includes $70,000.00 to conduct pre-schematic and conceptual design phases
of this project but the ultimate funding for this project must come from a successful referendum
vote. As previously indicated, the Region #16 Building Committee expects assistance from
design professionals to present the project to the two communities

This project is also eligible for State of Connecticut reimbursement. The Region #16 Building
Committee expects the selected firm to have extensive experience in the state reimbursement
process and all the related forms that will need to be filed to secure reimbursement. Please state
what experience the firm has in assisting school projects to secure funding.

OTHER NOTEWORTHY ITEMS:

1. The Region #16 community is comprised of the two communities of Beacon Falls and
Prospect. Region #16 is a K-12 system with one board of education and one
superintendent overseeing the whole system and the central office. The system currently
has one high school (9-12) located in Beacon Falls and one middle school (6-8) located in
Prospect, and three elementary schools (PK-5), one (PK-3) located in Prospect, one (3-5)
located in Prospect, and one (PK-5) located in Beacon Falls. The new proposed PK-5
Elementary school will replace the two Prospect schools.

2 As two relatively small suburban communities, budgetary concerns are very important.
The expectation is that a fiscally responsible building would be built and that full life
cycle analysis would be used to support significant projected expectations.

3 The committee is particularly interested in a firm’s experience with land issues and
analysis in New Haven County, Connecticut.

4. It is presently the intent of the committee if the referendum passes that the same design
firm or firms would continue with the next phase of the project. The committee,
however, reserves the right to make that decision following the referendum.

5. Region #16’s two elementary schools in Prospect are available for inspection at this time.
After a short list of firms has been developed, arrangements will be made for these firms
to tour the schools. Any available plans, specifications and drawings of the existing
schools and any environmental assessment will also be made available to the short listed
Design Teams.

6. Short listed firms will be required to provide bank references and/or financial statements
to show financial stability, evidence of proper insurance coverage and other resources
necessary to do a project of this magnitude.

7. The Regional School District #16 Board of Education prohibits harassment and
discrimination on the basis of race, color, religious creed, age, marital status, military or
veteran status, national origin, sex, ancestry, sexual orientation, or past or present
physical or mental disability in accordance with Titles VI, VII of the Civil Rights Act of
1964, Title IX of the Education Amendments Act of 1973, Section 504 of the
Rehabilitation Act of 1973, the Americans with Disabilities Act of 1991 and applicable
state laws.

8. The Region #16 Building Committee will be using the Qualification-Based-Selection


(QBS) process as outlined by the Connecticut QBS Council for the selection of design
firm.

9. The Region #16 Building Committee reserves the sole and absolute right to reject any
firm offering services which, in its opinion, does not meet the standard or quality
established by the Information Packet and the QBS process.

10. Conditional or qualified Qualifications shall not be accepted.

11. The Region #16 Building Committee requests that proposals be submitted electronically,
if possible, to the Business Office at e-mail hpotter@region16ct.org. If paper proposals
are submitted please send 12 copies. (Please note one copy of the firm’s brochure will
suffice.) All proposals (electronic or paper) must be received in the Region #16 Business
Office by 3:00 p.m. on January 31, 2011. Facsimile (FAX) proposals will not be
accepted by the Region #16 Building Committee under any circumstances.
