ACCOMPLISHMENT REPORT
MINUTES OF THE HOMEROOM PTA MEETING OF GRADE 7-HUMILITY
NO. DETAILS
1 The meeting was hosted by Ms. Rhea P. Glipo, grade 7 – Humility Adviser and with
the presence of the TLE subject teacher, Mr. Berne Baroman, as the documenter.
2 Checking of attendance was done. There were 27 out of 34 who were present.
There was an attendance sheet signed by the parents or guardians who joined the
meeting. (See attached attendance sheets in the MOV.)
3 ● Ms. Rhea P. Glipo opened the Homeroom PTA Meeting by introducing
herself as the Grade 7 HUMILITY adviser.
● Ms. Glipo shared the agenda of the meeting to the parents or guardians of
7-HUMILITY. The agenda included the following:
1. Documents (PSA/ NSO, Form 137 for the transferred in)
2. Student’s attendance (Absences of the learners)
3. Grading System
4. Subjects and Class Schedule
5. Classroom Rules (Rules for the Parents)
6. Election of PTA Officers
7. Proposed Classroom Project
8. Other Matters (Parents GC, School Uniform, Anecdotal Record
Template)
4 Ms. Glipo also shared the Vision and Mission of the Department of
Education. She reminded the parents and guardians to let their children
memorize the Vision, Mission, and Core Values.
She reminded the parents of those students who have not yet passed the
PSA or NSO that this is an important document to check the consistency of
the student’s data or records. She also reminded the parents of those who
are transferees to submit their credentials, or Form 137.
These are the following students who needs to submit their PSA document:
of absences, parents should call or text the teacher in the morning of the
absence and notify the teacher in writing upon the student’s return to
school. In the case of medical reasons, it would be better to present the
medical certificate.
Ms. Glipo mentioned the Grading System.
She also explained the class schedule of her advisory class. These are the
following:
7:15 – 7:30 (Every Monday) – Flag Ceremony
7:30 – 8:30 MTWTH (Ms. Glipo) - Mathematics
F (Ms. Soco) - MAPEH
8:30 – 9:30 MWTH (Ms. Soco) – MAPEH
TF (Mr. Mangubat) - AP
9:30 – 9:45 RECESS
9:45 – 10:45 MTWTH (Mrs. Cutab) – Science
F (Mr. Baroman) – TLE
10:45 – 11:45 MF (Mr. Bargayo) – ESP
TWTH (Mr. Baroman) – TLE
11:45-12:45 LUNCH
12:45 – 1:45 MTTHF (Mrs. Gapo) – English

1:45 – 2:45 MTWTH (Mr. Caraat) – Filipino

She also added that the students must bring foods for snacks and lunch.
She mentioned some rules for the parents to take note. These are the
following:
- Always follow-up your child
- Write excuse letter whenever your child is absent
- Do not tolerate your child
- Please ask the teacher on your child’s progress and status
- Cooperate with the adviser
She also informed the parents to join the group chat for communication
and announcement purposes. She also presented the school uniform and
the anecdotal record template. Anecdotal is not just for the student’s
misbehavior to draw attention but also to note the student’s progress and
achievements.
5 After which, the election of new sets of parent’s officers were conducted.
