 MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement executed by and between:

The BAROBO NATIONAL HIGH SCHOOL, a public secondary institution, with the
office address of Barobo, Surigao del Sur, represented herein by its School Head,
MARCELA G. MARTICION-ORTEGA, herein after referred to as “DONEE”;
- and -

The BARANGAY LOCAL GOVERNMENT UNIT OFFICIALS in the Municipality of

Barobo, an association with the address at , Barobo,
Surigao del Sur, represented herein by its Barangay Captain,
, herein referred to as
“STAKEHOLDERS”.

WITNESSETH: That,

WHEREAS, the DONEE, aims to implement the Gulayan sa Barangay at Tahanan to

maintain the Project FAITH (Food Always In The Home), promote and enhance vegetable
production in the home, increase KSA (knowledge, skills and attitude) among the learners on the
importance of backyard/home gardening, and educate parents on how to improve nutritional
status of their children;

WHEREAS, the DONEE, acknowledges the need for partners in the implementation of
Gulayan sa Barangay at Tahanan Project of Barobo National High School, Barobo Surigao del
Sur;

WHEREAS, the STAKEHOLDERS, will voluntarily provide assistance and other

resources to address the need of the school for the implementation of Gulayan sa Barangay at
Tahanan Project.

WHEREAS, the seedlings, shall be given by the DONEE to ensure the availability of
resources in all homes under the STAKEHOLDERS and not to burden them with the seedling
expenses;

NOW, THEREFORE, for and in consideration of the above premises and mutual
covenants, agreements and stipulations hereinafter set forth, the parties hereby agree jointly to be
partners on the implementation of the Gulayan sa Barangay at Tahanan Project with the
following stipulations:

I. ROLES AND RESPONSIBILITIES OF THE PARTIES

1. The DONEE shall:

1.1 Conduct orientation to the STAKEHOLDERS and its constituents about the
implementation of the project, Gulayan sa Barangay at Tahanan Project;

1.2 Provide seedlings to the STAKEHOLDERS to ensure that everyone gets to

participate on the project;

1.3 Closely monitor the progress of the gulayan until harvest.

2. The STAKEHOLDERS shall:

2.1 Help provide seedlings to the families under their barangay’s premises;

2.2 Assists the DONEE’s needs in terms of the profile and/or scheduling of visits;

2.3 Participates on the project by monitoring its constituents and provides the
required assistance for the purpose.

II. EFFECTIVITY OF THE AGREEMENT

This Agreement shall take effect upon the release of seedlings to the stakeholders for the
project implementation and shall be in effect until harvest and is automatically extended if
circumstances occur may be.
