I, RAMON ZARTIGA MADES, single, of legal age, natural born

Filipino citizen, and presently residing at 1148 E. Chestnut Avenue,
Orange California, 92867 USA do hereby
NAME/CONSTITUTE/APPOINT my daughter KATRINA JOY GABITO
MADES-DALAYAP, married to Lloyd Adrien C. Dalayap, of legal age,
Filipino citizen, and a resident of Cabittauran, Nueva Era, Ilocos Norte,
Philippines, to be my true and lawful Attorney-in-Fact, to do and perform
in my name, place and stead, the following acts and deeds, to wit:

1. To INITIATE/FILE the necessary action, petition or other

related case/s for the cancellation/correction of the entries in
the Certificate of Live Birth issued by the Philippine Statistics
Authority as well as that of the Office of the City Civil Registrar
of Manila of KATRINA JOY GABITO MADES, particularly to
correct the following entries:

1.a. In Item No. 4 (Place of Birth) &

(City/Municipality): From DR. JFMH to DR. JOSE
FABELLA MEMORIAL HOSPITAL, MANILA;
1.b. In Item No. 6 (Maiden Name of the mother): From
ROWENA G. GABITO to ROWENA GOSTOER GABITO;
1.c. In Item No. 9 (Name of the father): From RAMON
S. MADES to RAMON ZARTIGA MADES;
1.d. In Item No. 12 (Date and place of marriage of
parents): From 5/15/85 Ilocos Norte to NA (Not
Applicable) or blank;
1.d. In Item No. 14 (Informant): From ROWENA
GABITO MADES to ROWENA GOSTOER GABITO

2. To REPRESENT ME and TESTIFY on my behalf in all stages of

the proceedings of the said case/s;

3. To ENTER into stipulations of fact and law during the

preliminary conference and pre-trial proceedings;

4. To SIGN and EXECUTE the verification and/ or certification as

maybe required for the above-mentioned case/s;

5. To ENGAGE the services of a counsel to represent me in the

above-mentioned case/s; and

6. To EXECUTE, SIGN, RECEIVE, REQUEST AND DELIVER any

other papers, deeds or documents which may be necessary in
carrying out all of the foregoing.
