Statement of all Completed Government and Private Construction Contracts which are
similar in nature (SF-INFR-16)

Certificate of Site Inspection to be signed by the Municipal Engineer


Financial Document

Computation of Net Financial Contracting Capacity (NFCC)


equal to one hundred percent (100%) of the approved budget for the contract.

Class B Document

i.
Valid joint venture agreement, in case of a joint venture (if necessary)

ii.
Bid Security in the prescribed form, amount and validity period;

iii. Project requirements which shall include the following:


Duly signed Organizational Chart for the Contract to be bid (SF – INFR-44);

iv.
Duly signed Constructions Schedule and S-Curve

v.
Duly signed List of Contractor’s Key Personnel (Project Manager, Project Engineer,
Materials
engineer and Foreman) to be assigned to the contract to be bid, with their complete
qualification and experience data (SF-INFR-48)
vi.
Duly signed Manpower Schedule.

vii.
Duly signed List of Contractor’s Equipment units, (owned, leased or under purchase
agreement supported by availability of equipment from the equipment lessor/vendor for the duration of the
project (SFR-INFR-49), and
viii.
Duly signed Equipment Utilization Schedule

ix.
Duly signed Construction Safety and Health Program
x.
Sworn Statement by the prospective bidder or its duly authorized representative in the
forms
prescribed by the GPPB as to the following:
a. It is not “blacklisted” or barred from bidding by the GOP or any of its agencies, offices,
corporations or LGU’s, including foreign government/foreign or international financing institution whose
blacklisting rules have been recognized by the GPPPB ;

b. Each of the documents submitted in satisfaction of the bidding requirements is an


authentic copy of the original, complete, and all statements and information provided therein are true and correct;
c. It is authorizing the Head of the Procuring Entity or his duly authorized
representative/s to verify all the documents submitted;

d. The signatory is the duly authorized representative of the prospective bidder, and
granted the full power and authority to do, execute and perform any and all acts necessary and/or to represent
the prospective bidder in the bidding, with the duly notarized Secretary’s Certificate attesting to such fact, if the
prospective bidder is a corporation, partnership or joint venture;
e. It complies with the disclosure provision under Section 47 of the Act in relation to
other provisions of RA 3019
f. It complies with the responsibilities of a prospective or eligible bidder
provided in the PBDs, and
g. It complies with the existing labor laws and standards.

Note: Any missing document in the above-mentioned checklist is a ground for outright rejection of the bid

Checklist of Bid Requirements

2nd Envelope

FINANCIAL PROPOSAL

Financial Bid Form in accordance with the form prescribed in Section IX. Bidding Forms

Lump sum bid prices, which shall include the detailed engineering cost, in the prescribed
Bid Form;

Duly signed detailed estimates including a summary sheet indicating the unit prices of
construction materials, labor rates and equipment rentals used in coming up with the
bid;
and

Cash flow by the quarter and payment schedule


Note: Any missing document in the above-mentioned checklist is a ground for outright rejection of the bid
