12-1/4'' Trackmaster Tri-mill Twisted-Off
Scope:
Doha, Qatar

Purpose:
Highlight the need to follow whipstock milling guideline,
parameters and practices.

What Happened:
• 13-3/8'' whipstock was set without any issues.
• Start milling the window with standard parameters.
• After milling 7 ft of casing, WOM was increased to maximum
limit of the tri-mill. Milling torque at 40,000 ft-lb.
• Weight on mill was further increased to 48,000 klbs when drilling
rathole.
• No progress after 7691 ft.
• POOH, found out tri-mill twisted- off right below the follow mill.

Lessons Learned:
Immediate and Root causes.
• 12-1/4'' Tri-mill twisted-off right below the follow mill.

Root Cause
• Weight on mill was increased to more than double of the maximum recommended milling weight while
drilling of the rat hole.

Actions Required:
1. Stand down with the WIS Global Team
2. Parameter Road Map guidelines as per the operating instructions manual for the whipstock system provided to
the field hand during the brief must be strictly adhered to.
3. Any increase in parameters needs to be documented with an approval from the operations manager.
4. Qatar to have refresher training and certification of the field crew, this training include milling parameter
procedural adherence.
