Greetings!

I hope this letter finds you well. The undersigned is writing this letter in relation to a
building located at Purok Km. 13.5, Brgy. Taloc, Bago City, Negros Occidental, Philippines.
The original owner of the said building Rellie Lobrido, and after her death, her heirs has
been managing the operations of the said building. As of date, the said building was
registered under the name of my client, Resa Grace L. Torres with the business name “Lease
Hold Service”, and has been operating for almost fifteen (15) years.

Recently, when Ms. Lobrido applied for a renewal of the building’s business permit, it was
denied by your office due to an alleged infraction on the part of the building operator.
However, the records would show that there was no notice of infraction addressed to the
building owner or any of its representatives. Ms. Lobrido does not have any knowledge of
any infraction on the part of the decedent.

With these, on behalf of my client, I would like to request detailed information regarding
any infractions or violations that have been issued against the above-described building.
Moreover, I would like to request information about the current status of each infraction,
any penalties or fines that have been imposed, and any actions taken to remedy the
violations.

I appreciate your cooperation in providing the requested information. Thank you for your
attention to this matter. I look forward to receiving the requested details at your earliest
convenience.
