NOTES:

1. This Permit has been issued on the request of the Applicant and does not relieve the Business Partner or the Contractor appointed by them from any
legal and/or contractual obligations. All conditions mentioned in the permit/work authorization to be strictly adhered.

2. This permit is deemed to be cancelled and the works will be stopped immediately on any Health, Safety and Environment or other violations on site
noticed during the inspections or notified to the Facilities Management or DIFC security.

3. The applicant is fully liable for any penalties, rectification / cost implication of any damages and interruptions caused due to such violations by the
contractor and their subcontractors. The works can be resumed only after necessary corrective actions.

4. Any noisy or malodourous activities such as drilling, coring, painiting, drainage terminations etc. are permitted only after working hours and the
applicant to coordinate with the building security for the same to avoid stoppage of works.

5. Any works on existing building services and systems such as MDB/SMDB modifications/terminations, Fire Alarm & Fire Fighting, Emergency Lighting,
BMS, HVAC Controls etc must be carried out through the FM Service Provider only (IDAMA).

6. Applicant shall take adequate control measures & provide qualified fire watch(es) for monitoring affected areas in event of Fire Detection and/or
protection system impairment. Fire watch(es) shall be in place till impaired systems are re-instated to the origional state and impairment permit is
closed.

7. Applicant shall conduct HSE Induction, Tool Box Talks to their work force to brief work place and task related risk controls and building emergency
response. Such shall be recorded and evidenced to HSE/Security/Permit Issuer whenever required.

8. Kindly note this is Work Authorization Permit and not replacing any formal PTW for Work at Height, Isolation, Overriding of Fire and Life Safety
Systems, Hot Work, Lifting, Confined Space etc. A Separate Permit to be obtained.

9. The applicant/contractor/tenant is responsible to report all incidents and emergencies to DIFC/IDAMA/SECURITY and investigate/submit report as
applicable

10. If the job is not completed within the stipulated time, a fresh permit shall have to be raised and the copy of the permit marked “Applicant” shall be
handed over to the endorsee.

11. Common areas including elevators, fire systems and floor shall be protected against damages during mobilization, transportation of materials and
execution of works- any damages refer serial no. 3

12. Permit/Work Authorization timings to be strictly adhered

13. DIFC PMD signature or email approval shall be required for works that have a PO issued directly from DIFC.

14. Requests from other DIFC departments will require prior notification to DIFC PMD for works to proceed.
