This Adoption Agreement ("Agreement") is entered into between Jake Edward Roadilla Caparros

("Previous Owner") and Maria Raya T. Sablan ("New Owner") on this March 9, 2024.

Dog's Name: Maple

Breed: Shih Tzu
Gender: Male
Age: 1yr and 2months
Color: White, Brown and Black

Adoption Fee:

The New Owner agrees to pay an adoption fee of 5,000 pesos for the dog mentioned above. The payment
will be made in two instalments as follows:

a) The New Owner will pay an initial amount of 2,000 pesos on March 9, 2024, at 6:00 PM. This
payment will be made by Karren Delos Santos on behalf of the New Owner.

b) The remaining balance of 3,000 pesos will be paid by the New Owner to the Previous Owner. The New
Owner acknowledges that the payment may be made in full payment of remaining balance on or before
March 31, 2024 or partial instalment on March 31, 2024 then fully paid on or before April 30, 2024. The
new owner is fully committed to fulfilling the remaining balance by the specific date. If failed to pay the
remaining balance on date stated, the remaining balance will increase 1% every week.
Transfer of Ownership:

Upon receipt of the full adoption fee, the Previous Owner agrees to transfer ownership of the dog to the
New Owner. The New Owner will assume all responsibilities and liabilities associated with the dog from
the date of transfer.

Non-Interference:

The Previous Owner, Jake Edward RoadillaCaparros, agrees not to interfere with the New Owner's
ownership and care of the dog. This includes but is not limited to:
a) Not attempting to reclaim or take possession of the dog.
b) Not contacting or visiting the dog without the explicit consent of the New Owner.
c) Not interfering with the New Owner's decision-making regarding the dog's care, training, or medical
treatment.

Representations and Warranties:

a) The Previous Owner represents and warrants that they are the legal owner of the dog and have the
authority to transfer ownership.

b) The New Owner represents and warrants that they are capable of providing proper care, attention, and
a suitable living environment for the dog.

Health and Vaccination:

The Previous Owner represents and warrants that, to the best of their knowledge, the dog is in good health
at the time of transfer and has received all necessary vaccinations up to the transfer date.

Indemnification:

The New Owner agrees to indemnify and hold harmless the Previous Owner from any claims, damages,
or liabilities arising out of the ownership, care, or use of the dog after the transfer of ownership.

Governing Law:

This Agreement shall be governed and construed in accordance with the laws of the Philippines. Any
disputes arising out of or relating to this Agreement shall be resolved through amicable negotiations
between the parties. If the parties are unable to reach a resolution, they agree to submit the dispute to
binding arbitration in accordance with the rules of Philippine Dispute Resolution Center, Inc. (PDRCI)

Entire Agreement:

This Agreement constitutes the entire understanding between the parties and supersedes all prior
agreements, understandings, or representations, whether written or oral, relating to the adoption of the
dog.
Severability:

If any provision of this Agreement is found to be invalid, illegal, or unenforceable, the remaining
provisions shall continue in full force and effect.

By signing below, the parties acknowledge that they have read and understood this Agreement, and agree
to be bound by its terms and conditions.
