AT ANNEX BAY 7 AND BAY 8

I. SCOPE OF WORKS

ANNEX A - GENERAL REQUIREMENTS


[C] [NC] 1. MOBILIZATION/DEMOBILIZATION
1.1 Provision of equipment, temporary facility, portable electrical panel board with breakers including
extension wires (for all electrical connections; to be approved by authorized ABI personnel), etc.
needed by the contractor to commence the project

1.2 Site for temporary facility is subject to approval of End-user and Facilities Maintenance.

1.3 Area preparation/cleaning prior and after the project including the rehabilitation of the affected
areas.

1.4 Scrap from demolition and dismantling activities shall be disposed and hauled to ABI designated
disposal area, whereas, construction debris shall be disposed offsite by the Contractor.

[C] [NC] 2. SUPERVISION & MANAGEMENT


2.1 Provision of well-trained professionals to supervised the work activities every day at the work site
including a qualified Safety Officer with valid DOLE Certificate.

[C] [NC] 3. SAFETY REQUIREMENTS


3.1 Provision of necessary PPEs (such as hard hats, reflectorized safety vests, safety glasses, gloves,
safety rubber shoes and fall arrest system) for all manpower working at the site (rental only)

3.2 Provision of necessary reflectorized signage including: (1) submission of work site security plan
indicating the locations of signage within the work area during the entire work period, (2) stand-by
first aid medical kit and (3) provision of temporary enclosure.

3.3 Provision of work methodology & job hazard analysis submission prior to mobilization.

[C] [NC] 4. SCAFFOLDING WORKS


4.1 Provision of approved scaffold systems: guardrail system, planks and ladders.
4.2 Scaffolding system must be erected under the supervision of a NCII certified scaffold erector.

ANNEX B – CIVIL/STRUCTURAL WORKS

1.1 Scope (Actual conditions shall be verified by the Contractor on site.)

INSTALLATION OF SLIDING CURTAIN


AT ANNEX BAY 7 AND BAY 8 SCOPE OF WORKS
A. Submit agreed final Working Drawings and specification prior to start of the project to
Facilities and Maintenance and end-user.
B. Final Working Drawings and Specifications shall be indicated on the Plans to be approved by
the End-User, Pest Control & Facilities Maintenance Team.
C. Contractor shall verify all measurements.
1.2 Submittals
A. Submit Final Working Drawings with specifications to FM for approval prior to fabrication.

[C] [NC] 2. FABRICATION AND INSTALLATION OF SLIDING PLASTIC CURTAIN


2.1 Scope
A. Area preparation. Contractor to verify actual condition on-site.
B. Installation of plastic curtain 3 mm x 300 mm wide with 50 mm overlap between strips – both sides
C. PVC Strips riveted on aluminum flat bar and provide 5 mm clearance from floor surface.
D. Use schedule 80 for 2” GI Pipe Hanger that is coated with epoxy primer and epoxy enamel coat.
E. Installation of sliding curtain accessories. (railings, hangers, and rollers)
F. Installation of tubular frame with holder support.
G. Sealing of gaps.

2.2 Materials
Plastic Curtain
A. PVC Strip Curtains, clear, 300mm x 3mm, 50 mm overlap each side.
B. GI Pipe 2” diameter schedule 80.
C. MS Plate ¼” thk x 4” x 4”
D. Aluminum Flat Bar 1/8” thk x 1-1/2”
E. Consumables (Bolts and Nuts)

Sliding Accessories
A. GI Pipe 2-1/2” hanger support, schedule 80.(Trolley with a carrying capacity of 80kg)
B. Railings
C. Rollers, Heavy Duty
D. Tubular Frame, 2” x 4” x 1/16” with holder.
E. 4mm SS Heavy Duty Chain connected with handle.
F. Consumables (including sliding door puller)

Sealing of gaps
A. PVC Strips, 3mm thick (verify from existing)
B. Aluminum Flat Bar, 1” x 3mm thk
C. Rivets, 1/8” x ¾”
D. Consumables.

[C] [NC] 3. PAINTING WORKS AND RESTORATION WORKS


3.1. Scope
A. Painting of sliding curtain top rail including GI Pipe.

3.2. Materials
A. Epoxy Primer by Davies
B. Epoxy Enamel by Davies (2 coats)
C. Consumables

II. CONTRACTOR’S RESPONSIBILITIES


[C] [NC] A. Contractor is encouraged to study the scope of works carefully and verify actual site condition before
submitting bid proposal.

All unforeseen works during inspection that are necessary to complete the project shall be
guaranteed by the contractor.

[C] [NC] B. The contractor shall submit to Facilities Maintenance AGREED FINAL WORKING DRAWINGS WITH
SPECIFICATIONS prior to the fabrication & installation activity.
All project layouts and drawings need to be submitted both hard and soft copy prior to the start
of the activity for verification and approval.
[C] [NC] C. The contractor shall coordinate and schedule with the end-user all work activities inside the production
area. No worker will be allowed to work without his standard PPEs (including hair net) fully worn.
[C] [NC] D. The contractor shall process all necessary permits and corresponding expenses prior to construction of
the project.
[C] [NC] E. GMP & Food Safety Orientation and Awareness.
[C] [NC] F. Contractor is responsible for hauling of scrap, garbage for disposal offsite.
[C] [NC] G. Contractor is liable from any damage that might incur to ABI properties.
[C] [NC] H. Contractor to submit one (1) original copy and two (2) photocopies of the following:

For Progress Billings:


1. Billing Request/Statement
2. Purchase Order
3. Photocopy of Invoice
4. Accomplishment Report (S-Curve and Statement)
5. Progress Photo (before, during and after)
6. Scope/Design Change
7. Delivery Receipt
8. Bill of Quantity

For Final Billing:


1. Certificate of Accomplishment/Project Acceptance
2. Purchase Order
3. Photocopy of Invoice
4. Accomplishment Report (S-Curve and Statement)
5. Progress Photo (before, during and after)
6. Scope/Design Change
7. Delivery Receipt
8. Bill of Quantity
9. Aligned BOQ and Delivery Receipts
10. Certificate of Warranty
11. Logbook
12. Signed and Sealed As-Built Plan in A3 & A4 size and soft copy in USB storage.

III. SECURITY AND SAFETY REGULATIONS


[C] [NC] 1. All works must comply with all OSHS and local Safety Rules and Regulation
[C] [NC] 2. All equipments used by the contractor inside the company premises must be in good working
operation and maintained during the life of the contract. Any equipment which does not meet the
requirements must be repaired immediately or removed from the company.
[C] [NC] 3. In the event that an accident involving the contractor’s personnel, equipment, or operation occurs
which result in an injury or damage to either company or contractor’s property, the contractor shall
notify the ABI Project-in-charge or security guard on duty

AT ANNEX BAY 7 AND BAY 8 SCOPE OF WORKS
[C] [NC] 4. Provide Safety Officer and First Aider certified by DOLE and Philippine Red Cross, respectively,
Project Safety Officer to attend weekly safety meeting.
[C] [NC] 5. PPE's and ID's are to be worn at all times; Identify and use designated eating areas, and toilet
facilities.
[C] [NC] 6. Accomplish & submit Work Safety Permit weekly and Hot Work Permit daily.
[C] [NC] 7. Being a Food Company, all workers of the contractors should be in good groom, clean, cut & shaven.
[C] [NC] 8. All project workers should undergo Security & Safety Orientation prior to start of work. Currently, the
orientation is held every Saturday morning, 8:00 a.m.
[C] [NC] 9. All trashes shall be removed by the contractor from the worksite on daily basis.
[C] [NC] 10. All work areas must be kept clean as possible to prevent accumulation of excess debris.
[C] [NC] 11. Only the Project Engineer/Project-In-Charge, Safety Officer and Foreman are allowed to bring and
use the cellular phones.
[C] [NC] 12. Strict compliance to DOLE Department Order 13 (Construction Safety) by the contractor.
[C] [NC] 13. All Project workers entering the ABI Complex should be in complete PPEs. They shall enter through
Gate 2 (Tanduay Gate) and undergo frisking and inspection by the security guards.
[C] [NC] 14. Each contractor shall submit a list of workers, including project engineers, supervisors and safety
officers, to the EHSSD office indicating the regular working days and time of work for the duration of
the project. This should be countersigned by the Facilities Maintenance.
[C] [NC] 15. Any overtime work on any particular day or week, the contractor should seek clearance from the
Security Section of the EHSSD through a written memo of the overtime with the persons who will work
overtime, location and tasks to be done. This should be approved by the Facilities Maintenance
Supervisor-In-Charge and be submitted to the EHSSD office on or before 2:00 pm on the day the
overtime work is to be done. Overtime work is only allowed until 11:00 pm, the closing time of Gate
2. Any overtime work beyond the said time or overnight work shall be approved on a case-to-case
basis.
