Respected sir,

I am writing this letter with a kind request to provide the hospital with adequate and
proper new medical equipment / repairs (2 Lists attached).

Kindly note that an assessment of all hospital equipment by third party was done on
24/05/2024 along with MSDS Consultant, the undersigned, Hospital Administrator, OT
Manager and OT In charge.

As we are going to implement MSDS and shall equipt our hospital with equipment as per
KP HCC Regulations. This mater is very much of importance. This issue needs to be paid
attention as soon as possible.

You are hereby kindly requested to float a tender in the market through purchasing
committee for quotation. So we can go ahead with repair and purchase of the attached
equipment.

I hope you will acknowledge this situation and will resolve the matter soon. Thanking
you in anticipation.
