 I, Mrs. PUSPA DEVI KALANI, aged about 80 years, wife of Late Nathmal
Kalani, resident of Marwari Patty, Jorhat, P.O & P.S. Jorhat, District Jorhat, Assam,
do hereby solemnly affirm and declare on oath as hereunder–

1. That I am a citizen of India and a permanent resident of aforesaid locality.

2. That my name is PUSPA DEVI KALANI.

3. That I am also popularly known as PUSPA KALANI.

4. That in some of my documents / official records my name has been recorded

and appears as PUSPA DEVI KALANI and in some documents and official
records my name appears and recorded as PUSPA KALANI.

5. That now I hereby declare that both the names PUSPA DEVI KALANI and
PUSPA KALANI are the names of one, same and single person. i.e. me.

6. That this affidavit is sworn in to prove the above mentioned facts before the
concerned authority as and when required.

7. That all the statements made above are true to my knowledge, belief and
information.
